<p>Five to eight percent of all children in the United States have a speech sound disorder (SSD).</p>

<p>Children with SSD have difficulty producing sounds of their target language system. Some of these children also have difficulty perceiving and categorizing speech sounds. It is presently unknown what underlying mechanisms might account for the communication problems children with SSD encounter. One possible explanation is that children with SSD cannot produce speech sounds correctly because they have poorly specified phonological representations, which are the result of inaccurate speech sound perception. Thus, speech sound production errors may stem from imprecise speech perception and its resulting sparse phonological representations.</p>



<p>We propose to use electrophysiological measures (event-related potentials, ERPs) to examine how phonological representations and their associated auditory sensory responses change in conjunction with two traditional speech treatment approaches: one that does and one that does not involve auditory feedback with Forbrain. A better understanding of phonological representations and the auditory sensory system in children with SSD will inform how speech evaluations and treatment are best conducted by speech-language pathologists.</p>



<p>Our major objective is to characterize how traditional speech treatment (with and without auditory feedback with Forbrain) alters the auditory neural responses to sounds targeted in treatment. Since SSD has a relatively high incidence, a good understanding of its underlying nature can inform its clinical management, which could ultimately improve the lives of the affected children and their families. Specifically, children who receive treatment for their SSD have better long-term social, academic, and communicative prognoses than those who do not receive treatment. Here, we propose to use ERP measures to assess auditory sensory processing of speech syllables in children with SSD.</p>

<p><strong>Aim:&nbsp;</strong><strong>To assess how ERP auditory sensory responses evoked by English syllables change over time. This will allow for a better understanding of how the neural mechanisms associated with speech perception in SSD change after a period of traditional speech treatment, or intervention that combines traditional treatment and auditory feedback. It is predicted that the speech treatment program that includes auditory feedback with Forbrain® will enhance the phonological representations of children with SSD above and beyond what occurs with traditional treatment alone, resulting in ERP amplitude and latency waveform changes.</strong></p>



<p><strong>Treatment:</strong></p>



<p><strong><u>Speech-sound treatment for children with SSD</u>&nbsp;</strong>&#8211;&nbsp;Treatment will follow proven Evidence-Based Practice (EBP) methods of previous treatment studies of children with SSD (Cummings &amp; Barlow, 2011; Gierut et al., 1996; Gierut &amp; Morrisette, 2010; Morrisette &amp; Gierut, 2002). A single-subject staggered multiple baseline design will be used in this treatment program, as it has been shown to be useful in the study of the treatment of communicative disorders (Connell &amp; Thompson, 1986; McReynolds &amp; Kearns, 1983; McReynolds &amp; Thompson, 1986). Following procedures for this design, the children with SSD will be randomly assigned to one of two treatment conditions: traditional speech treatment alone (Tx) or traditional speech treatment with auditory feedback training via Forbrain® (AFTx). Every child will be evaluated in a baseline period in which no treatment was provided, followed by speech treatment.</p>



<p>Consistent with efficient and effective Evidence-Based Practice procedures used previously by the PI (Cummings &amp; Barlow, 2011) and others (Gierut et al., 1996; Gierut &amp; Morrisette, 2010; Gierut &amp; Neumann, 1992; Gierut, 1992; Morrisette &amp; Gierut, 2002), treatment will be delivered in two phases: Imitation and Spontaneous Production. Treatment will be provided two times weekly in 1-hour sessions, for up to 14 sessions. Each treatment sound will be targeted through the production of five words that will be initially introduced to the child using a storybook reading format. During the Imitation phase of treatment, each child will be shown a picture of the target word and will be asked to repeat the clinician’s verbal model until achieving either a pre-established performance- or a time-based criterion, whichever comes first. Specifically, imitation treatment will continue until a child maintains 75% accurate production of the treated phoneme over two consecutive sessions (i.e. performance-based criterion) or until seven consecutive sessions are completed (i.e. time-based criterion) (Gierut et al., 1996). During the Spontaneous Production phase, each child will produce the treated phoneme without a model. In other words, the words targeted in treatment will be elicited by having the children name pictures, label objects, retell stories, and so on. This phase of treatment will continue until the child maintains either a performance-based criterion of 90% accurate production of the treated phoneme over three consecutive sessions or a time-based criterion of seven consecutive sessions, whichever comes first (Gierut et al., 1996). It is expected that children will spend approximately 3 months in treatment.</p>



<p><strong><u>Auditory feedback training for children with SSD</u></strong> &#8211; Half of the children with SSD will receive auditory feedback via Forbrain during their twice-weekly speech treatment sessions. Each child will wear the Forbrain device for 20 minutes during each treatment session. Specifically, the child will put on the Forbrain device immediately after the reading of the treatment story and then wear it continually for the subsequent 20 minutes of treatment. This selected period of time in treatment sessions typically involves the shaping/elicitation of the treatment sound in isolation, as well as the production of the treatment words in drill play activities. <p><strong>Teachers and Parents review Forbrain for improving Attention and Memory</strong></p>

<p><strong>Occupational Therapist uses Forbrain technology to improve therapy outcomes for children with Autism Spectrum Disorder (ASD)</strong></p>



<p>Recently, I had the opportunity to talk with an Occupational Therapist about how Forbrain has worked in her practice, and why she considers it a staple in her therapy toolbox.</p>



<p>Kara Tavolacci, OTR/L, works with children with Autism Spectrum Disorder (ASD) and&nbsp;<a href="https://blog.forbrain.com/sensory-processing-disorder-subtypes-and-activities-that-help">Sensory Processing Disorder</a>&nbsp;(SPD). Children with these disorders are dynamic clients that require a therapist who is “on the ball.” Each child has different challenges and different therapy needs, which requires exceptionally personalized treatment plans. Despite her clients’ unique abilities, Forbrain has become a standard complement—and sometimes basis—of treatment plans in Kara’s practice.</p>



<p>Kara has been working as an Occupational Therapist for nearly fifteen years. She found her niche working with the pediatric population and soon after, joined many other clinicians who use Forbrain regularly in their practice. Now she practices at the Tomatis Sensory Center in Paris.</p>

<p>Kara explains that the success she has experienced using Forbrain in her practice comes mostly from its variety of applications. If a child has an apparent speech or language delay, she may test the use of the Forbrain during the initial evaluation to see how a child responds. In the case of a child without a speech or language delay, Kara can still use the device and observes benefits like:</p>

<h3>When is a Child with ASD a Good Candidate for Forbrain?</h3>
</div>


<p>Kara tells me that all patients that walk through the door are potential candidates for use of Forbrain. “Usually, within the evaluation, I’m trying it on to see how they respond to it.” Even if the child isn’t keen to it on the first try, she’s usually talking about it and attempting it within the first few sessions.</p>



<p>It’s a little more obvious when a child presents with a significant speech or language delay. In these cases, kids are usually working with a Speech-Language Pathologist as well, and Kara will recommend that a family take the device home to use in their speech sessions. This multi-disciplinary approach is a model that is known as invaluable to success in treatment.</p>



<h3 class="wp-block-heading">How to Introduce Forbrain to a Client</h3>



<p>When introducing the Forbrain to a client for the first time, Kara likes to know her audience. Sometimes parents are interested in the technology of the device. They want to know about the dynamic filter and audio-vocal loop. Sometimes they just need to know that it helps enhance their child’s voice in a natural way, leading to improved brain function. Either way, taking the time to address a parent or client’s questions about the device before using it will help improve understanding and willingness.</p>



<h3 class="wp-block-heading">How Children with ASD Respond to Forbrain</h3>



<p>“Children are really into it!” says Kara about&nbsp;<a href="https://blog.forbrain.com/top-3-home-based-tools-for-communicating-with-a-child-with-autism-spectrum-disorder-asd">children with ASD</a>&nbsp;who use the Forbrain. “They like to pretend they are rock stars.” Some children, on the other hand, may need some time to adjust to the experience of connecting and controlling their voice. “The voice is very powerful so it can be overwhelming for kids when they first experience connecting with their voice. Don’t be afraid of that, don’t give up on it; maybe try it for two minutes then another two minutes until they get comfortable with listening to their own voice.” Kara recommends putting it on for a short period of time and increases the duration to build acceptance.</p>



<p>For children who are reluctant at first and are developmentally appropriate, the use of distraction is an option. Kara will engage the child in motor activity (such as bouncing on a ball or swinging on a swing) and quickly place the Forbrain on them and talk about what is happening: “One, two, three, go!”</p>



<h3 class="wp-block-heading">Using Forbrain During Therapy Sessions to Improve Learning</h3>



<p>Occupational therapists like Kara provide therapies tailored to the unique needs of the child with ASD and SPD or other motor-sensory difficulties. An example of how Forbrain can easily assimilate into these activities. It is by following a therapy activity with an oral review. During the oral review, the client recalls and discusses the activity that he performed while wearing Forbrain headphones. Kara says that this strategy has a noticeable effect on recall and attention.</p>



<p>Kara appreciates the importance of following the child’s lead when it comes to treatment goals. She’s always looking to increase the time that a child uses the Forbrain but takes seriously the ability and willingness of the child to be challenged in a productive way.</p>

<h3 class="wp-block-heading">Older Children and Teenagers with ASD</h3>



<p>Kara has the opportunity to work with children of all ages. With some of the older children, she uses the Forbrain to help improve vocal control, oral reading, and to increase spontaneous verbalizations with people in their environment. Sessions include reading aloud while sitting up against a wall (posture is important!) or</p>



<h3 class="wp-block-heading">Home Program</h3>



<p>A good home program is imperative to improve the generalization of skills learned in therapy sessions. This is why Kara likes to do patient and caregiver education during sessions: “I like to have the parents put the device on [themselves] during the session so they can get acclimated to it: it’s a much better way for them to learn how to use it.”</p>



<p>A typical home program includes recommendations for how long / how often to use the Forbrain, as well as explaining to parents that it’s not an&nbsp;<em>additional</em>&nbsp;activity they need to add to their schedule. “I tell the parents that if an older child is doing homework—especially reading homework—they can do it with the Forbrain. [It’s] so great because it’s flexible and easy to use!”</p>



<p>For younger children, a floor-time model or play-based activity is best. Children tend to increase their spontaneous vocalizations while wearing the Forbrain even when playing alone. Offering child verbal activities that include turn-taking and eye contact with another person. It helps the communication become more&nbsp;<em>functional</em>&nbsp;(mimicking the skills they would use in a real-life communication-based scenario).</p>

<p>Building a successful caseload when working with children with ASD can be a challenge in that there is no one-size-fits-all treatment plan. <p>“Personally, I use it myself. I’m not a big fan of public speaking or presentations. I practice my speeches with Forbrain and I also use it because I’m trying to learn French at the moment. <p>The sound starts its journey when captured by your outer ears, the two formations on either side of your head, traveling in air through several structures within the ear, then translated by your nervous system and your brain for use as information to live in the world. In those with normal hearing, sound waves enter the outer ear, travel through the middle ear to the ossicles, three small bones, which transmit sound vibrations to the cochlea vestibule nerve in the inner ear. The&nbsp;<a href="https://en.wikipedia.org/wiki/Cochlea">cochlea</a>&nbsp;vestibule’s job is to convert these sounds into electrical impulses and send them along the auditory nerve to the brain.</p>



<p><strong>Bone Conduction Canal</strong></p>



<p>There is another fascinating way our body processes sound. Sound waves are also transmitted through the bones in our heads! When our bones vibrate, the sound is sent directly to the cochlea vestibule nerve in the inner ear. This sound transmission directly through our bones is similar to traveling through the eardrum and middle ear. This vibration also creates nerve impulses and connects sounds to our brains. This is called bone conduction!
